Do I Need to Fill Out Paperwork in Advance?

If you arrive early for your first appointment, you can fill out the paperwork before we begin. However, we strongly encourage patients to download and complete their new patient forms in advance, especially if you have chronic health problems you feel may be related to conditions in your mouth. This will give you more time to be thorough with your health history in particular. The more information Dr. Bybee has, the better he can address your needs.

Do I Really Need to Have X-rays Taken?

All new patients are required to have a current full-mouth series of x-rays and a CBCT scan so we can fully evaluate your current dental health and have a baseline against which to track changes over time. Too often, x-rays from another office are out-of-date or of insufficient quality for our – and your – needs.

To limit your radiation exposure, we use only digital imaging. We also provide you with 120 mg of melatonin before taking the CBCT scan, as melatonin is known to limit free radical damage from the radiation.

What If I Need to Cancel or Reschedule My Appointment?

We ask that you give us at least 24-hours notice if you won’t be able to make it to your scheduled appointment. This gives us an opportunity to offer your spot to another patient who has been waiting for an opening.

What If I Miss My Appointment or Give Less Than 24-Hours Notice?

If you miss more than one appointment without notice, you may not be able to reschedule.
